New Patients

Tammy Graham-Fortune, DMD is always accepting new patients. There are a few important steps that must be taken before your first visit so that our staff can familiarize themselves with you and your oral health needs. This page contains our new patient forms that you will need to get started. They are securely sent to our office manager.

Dental Insurance

We are in-network with most major PPO dental insurance plans, including  Medicare plans to serve our senior patients. 


However, we are more than happy to accept out-of-network plans! 

You can still save quite a bit even with an out-of-network practice, our office manager will easily be able to maximize your benefits no matter your provider, meaning you can always come see us and know that you’ll be able to save.

Affordable Dental Care for Everyone

Many of our patients do not carry dental insurance, however, that does not prevent them from getting the care they need. Dr. Tammy Graham-Fortune promises affordable rates and honest treatment because we are a family-owned, 2nd generation practice.  She will customize your care based on your individual needs and dental health goals. We can prioritize your care to fit in your budget.

Care Credit

Sometimes treatment cannot wait and that’s why we offer CareCreditâ„ , a financing option that allows you to break up the cost of your dental work into low monthly payments. These payments can be broken up into 6, 12, 18 or 24 month financing options with no interest. Call our office to find out more information.
